The 34-year-old All Black had featured in every game before he collapsed in the first match at Parc y Scarlets, against Munster in the Magners League.
This weekend he partners Phil John and hooker Ken Owens in the front row. Head coach Nigel Davies has opted for youth in the second row, with 21-year-old Lou Reed and 23-year-old Dominic Day. Flankers are new Wales call-up Rob McCusker, aged 23, and Johnathan Edwards, 25.
Ever present No 8 David Lyons keeps the captaincy with Simon Easterby starting on the bench with knee surgery fresh in the memory.
Lions tourists Matthew Rees and Stephen Jones are rested along with Wales tourists Jonathan Davies, Dafydd Jones, Mark Jones. New Wales squad backrow Josh Turnbull starts on the bench.
Lee Williams keeps his wing berth after a two-try man-of-the-match performance against the Blues on Wednesday at Cardiff Arms Park.
Nigel Davies said: "It was a fabulous performance by Lee in Cardiff. He is a world-class sevens player, he's got that ability to beat players, he is very quick, he understands the game very well and he's one of those players that can play in a number of positions. He showed what a great talent he is.
"What was equally encouraging was that there were a number of other really big performances out there on Wednesday. Dan Evans kicked fabulously well and put us in good areas; a good number of our younger players showed what they are capable of and really stood up.
"We're not going to congratulate ourselves on qualifying for Europe last Wednesday but I think the youngsters that night showed the enormous potential within this squad. They have got to have belief now and we've got to move forward.
"We've invested heavily in them this year; it hasn't always worked for us but in Cardiff we saw some encouraging signs for the future.
"Our defence was superb on Wednesday - probably the best it's been all year and we created some nice tries as well.
"It goes without saying that we want to improve on this season. The youngsters have been blooded this year and we saw them really come through for us this week. "We have to kick on from here; the boys can take a lot of confidence out of Wednesday and we have to put in a big performance again against Ulster.
"This season has been tough for the supporters, but the Cardiff result gave them something to shout about tonight and hopefully we can do the same against Ulster."
Simon Easterby: "We've struggled this year to get the results and we left ourselves a lot to do come the final few games of the season. We managed to pull a fantastic performance out of the bag in Cardiff so it's a stepping stone to better things next season.
"Our kicking game was outstanding on Wednesday and we to ok our chances; probably what we haven't done all season is to take those chances when we've gone away, so it was a fantastic effort.
"There's less pressure now for the Ulster game but we set a standard in Cardiff and hopefully we can continue that against Ulster. We want to finish on a high and take that form into next season, so it's important that we don't sit back and stop playing rugby. We have to win the game and win well this weekend."
